Nuremburg Laws?

I hope someone can help me. During the time of Hitler in Germany, German citizens had to prove their German ancestry. There was a special form that they had to fill out proving their ancestry for at least 3 generations. Can anyone help me with the name of this form?
Where would one obtain copies of these forms pertaining to one's family?

The only information I have been able to find on the Internet to date has been the Nuremburg Laws, which pertain to the purification of the German race.

besides "Ariernachweis" it was also called "Abstammungsnachweis".

As far as I know they all were kept within the Family.
