1) yes, Jan is also the Platt form of Johann, e.g. I always called my uncle 'Jan'; his official name was Johann.
2) Harm is a common Platt version of Hermann, the patronymic form usually is 'Harms', the -sz. version was not used around here, AFAIK. To me it does definitely sound Dutch.
